Adventure in Nepal runs it, and 9 things are covered by the price. It can be cancelled free of charge.

The price covers hotel pickup and drop-off, Professional, friendly English speaking trekking guide, Guide: food accommodation and all expenses and Trekking permit and all official document. It does not cover food and drinks (Avaliable for purchase), Gratuities (Optional) and Incase an emergency rescue (Cover by your travel insurance if you have), so budget for those separately. Two travellers pay $795 together rather than $521 each, because this is priced per party and not per head.
